Yes you can add tomatoes, they compost down quite quickly only problem with them is the seeds tend to grow :) :) :)

not sure why your worms should look as if they are cut in half, never come across this before ?
What sort of wormery is it, does it smell we need to find the reason why your worms are dying

If it doesn't smell then its probably ok.
at 3 months the wormery is still in its youth, don't worry too much about worms in the lid unless there are hundreds of them, worms like to explore (especially at night) also when its thundery or wet they move to the surface naturally.

keep an eye on them to make sure their condition doesn't worsen
